you can try capturing the source of the form submit. this needs to be done by having a hidden form field and by capturing that using a query string value (from the link to the page/form in the email eg: www.google.com?source=email1). the other email/campaign will have another value of source. i think it would make reporting/segmentation easier if you had new forms/pages for each campaign, but you can try the above mentioned apporach
